TEAM DAILY ATHLETICS – LIABILITY WAIVER AND RELEASE

Assumption of Risk
I understand that participation in basketball training, fitness, and athletic activities with Team Daily Athletics / Team Daily AAU involves certain inherent risks, including but not limited to muscle strains, sprains, fractures, and other potential injuries. I voluntarily assume all such risks associated with participation.
Release of Liability
In consideration for being allowed to participate in Team Daily programs, I release and hold harmless Team Daily Athletics, its coaches, staff, volunteers, and affiliates from any and all claims, demands, or causes of action arising out of injury, loss, or damage sustained during participation, whether caused by negligence or otherwise.
Medical Authorization
I authorize Team Daily staff to provide or seek medical treatment in the event of an emergency involving myself or my child. I understand that I am responsible for any medical expenses incurred as a result of participation.
Photo & Media Release
I grant permission for Team Daily Athletics to use photographs, videos, or likenesses of myself/my child for marketing, social media, and promotional purposes without compensation.
Code of Conduct
I agree that myself/my child will act respectfully toward coaches, teammates, and facilities, and follow all rules and safety instructions. Team Daily reserves the right to suspend or dismiss participants for unsafe or disrespectful behavior.
Acknowledgment
I have read and understood this waiver. I acknowledge that by signing electronically, I am voluntarily waiving certain legal rights, including the right to sue Team Daily Athletics, its staff, and affiliates.
